Patient and Family Advisory Council

|
At Tufts Medical Center and Floating Hospital for Children we are pleased to announce our newly formed Patient and Family Advisory Council (PFAC). Tufts MC is committed to partnering with patients and families to integrate their voice and perspective into how we deliver care. Our PFAC serves as the collective voices of our patients and their families. We strive to demonstrate and adhere to the principles of advocacy, collaboration, communication and participation in the work we collectively execute.
Our Patient and Family Advisory Council will seek the viewpoints of patients and families directly into the planning, delivery, and evaluation of health care. The goal is to achieve the best outcomes for patients through a closer, more mutual, shared relationship between the health care professionals and patient and families. Patient–Family centered care is our model of care and applies to patients of all ages and practiced in all health care settings at Tufts Medical Center and Floating Hospital for Children. As our mission states: “Our patients and their families are at the center of everything we do.”
Beginning in the fall of 2010, Tufts Medical Center embarks in a formalized manner to incorporate the patient and family care experience into all aspects of care and services. Six patients and/or family members have been selected to participate in our inaugural council with four Tufts MC staff members. They represent all areas of care and are representative of our demographics. Our Patient and Family Advisory Council will serve as one important vehicle for that collaboration and voice so we collectively enhance the patient and family care experience.
