What to Bring
When you arrive at Tufts Medical Center or the Floating Hospital for a doctor’s visit, please make sure to bring the following:

The name, address and phone number of the physician who referred you to Tufts Medical Center or the Floating Hospital

The name, address and phone number of your primary care physician

Insurance information, including ID cards, forms and policy numbers

Appropriate HMO referral authorization forms and/or policy numbers

Medical records, including x-rays, lab reports and any written medical history
-
A list of any medications you or your child is taking, including dosages

A list of any allergies to medications

You or your child’s red Tufts Medical Center hospital card, if he/she has been to the hospital before

Your own questions to discuss during your visit

Arriving and Registration
Upon arrival at Tufts Medical Center or the Floating Hospital, you must register at one of the outpatient registration/ information desks if you or your child:

Is a new patient
-
Is covered by MassHealth
-
Has an expired hospital identification card, or
-
Has had a change in address, insurance or other pertinent information since your last visit
You can check in at any registration desk within Tufts Medical Center or the Floating Hospital. Registration/information desks are available at the following locations:

About Your Visit
If this is you or your child’s first visit to Tufts Medical Center or Floating Hospital, please plan to arrive about half an hour before your scheduled appointment in order to register and complete necessary insurance and billing information. Please allow extra time for commuting at rush-hour or around matinee theater or dance performance times in the nearby Theater District.

Advance Preparations
Some tests or procedures require patients to fast, eat (or avoid) specific foods or make other accommodations -- such as refraining from driving -- to get the best possible test results or to ensure patient safety. Be sure to check with the clinic and medical staff about whether you or your child will have any specific physical or food-related limitations in connection with your visit.

Pharmacy
The outpatient pharmacy is located on the street level of Floating Hospital for Children, 755 Washington Street. The pharmacy can fill prescriptions written by any hospital physician. Approved prescription refills can be called in to
617-636-5385 and picked up at your convenience.
The pharmacy is open Monday through Friday, 9 a.m. to 6 p.m. Payment or co-payment for pharmacy prescriptions is due at time of pick-up.
Information Centers
If you or your child requires special assistance while at Tufts Medical Center or Floating Hospital, such as directions, wheelchair assistance or information of any kind, please pick up the phone at the nearest Information Center and dial the number indicated for assistance. Information Center phones are located at various entrances and key points around the hospital. The phones are clearly marked and have a "you are here" map nearby.
