Clinical

Dr. Inker's clinical interests are general nephrology, detection and treatment of CKD, glomerular filtration rate and geriatric nephrology, HIV-associated kidney disease, and kidney stones. She is the Director for Quality Improvement for the Division and Director of the Kidney and Blood Pressure Center.

Research

Dr. Inker’s primary research interests are in epidemiology and outcomes related to CKD.  Her major research interest is in the estimation ad measurement of glomerular filtration rate (GFR).  She was the co-principal investigator of the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), a collaborative effort to use pooled individual patient data to address central questions in the epidemiology of CKD, including developing improved glomerular filtration rate (GFR) estimation equations, using these equations to refine prevalence estimates for CKD from National Health and Nutrition Examination Survey.  The principle outcomes of this work are new estimating equations for GFR  the CKD-EPI creatinine equation, the CKD-EPI cysatin C equation and  the CKD-EPI creatinine-cystatin C equation.  All of these are more accurate than the cucrently most commonly used equation, the MDRD Study equation. The creatinine-cystatin C equation is more precise than either the CKD-EPI creatinine or the CKD-EPI cystatin C equation. Dr. Inker is the recipient of a K23 career development award to study issues related to kidney function and aging, as well as two other awards, the American Society of Nephrology American Specialty Professors (ASN-ASP) Junior Development Grant in Geriatric Nephrology and the DCI-Paul Teschan Research Fund. Both projects are designed to investigate the validity and reliability of GFR measurements. Dr Inker has a research grant from Gilead, Inc. to measure GFR in HIV-positive patients to evaluate the accuracy of creatinine- and cystatin C-based estimates of kidney function in this population.  Dr. Inker is studying kidney function in the elderly and in racial and ethnic groups through NIH funded grant to measure GFR as ancillary studies to the Aging Genetic Environment Susceptibility- Reykjavik Study (AGES-Reykjavik Study)  and Multiethnic Study of Atherosclerosis (MESA).  She is now an investigator in the NIDDK CKD-Biomarker Consortium, a collaborative effort to discover and validate new markers for CKD.  Through this and other projects, Dr Inker is investigating novel biomarkers of kidney function. 

Another major research interest is investigation of alternative endpoints for kidney disease outcomes in clinical trials. As part of the CKD-EPI project, Dr Inker evaluated changes in proteinuria as a surrogate marker for kidney disease outcomes in clinical trials. This work is continuing in the study of proteinuria as a surrogate marker for IgA nephropathy through a grant from Pharmalink. Dr Inker will be evaluating alternative creatinine based endpoints for a conference sponsoered jointly from the National Kidney Foundation (NKF) and Food and Drug Administration (FDA).

Policy - Guideline Development and Implementation

Dr. Inker is also interested in research and policy related to implementation of CKD Guidelines. She works with NKF leadership and national organizations (including the NIH's National Kidney Disease Education Program - NKDEP) to implement CKD guidelines, including issues related to GFR estimation and revisions to diagnostic codes for kidney disease.
