TEACHING ACTIVITIES AND ACHIEVEMENTS

Administrative

Program Director, the Residency in Surgery – 1992-1997

ACGME approved residency in general surgery - 4 categorical slots for the 6 year program and 9 preliminary slots per year. Responsible for all aspects of the program including application review, interviews, match list, department orientation, rotation schedules, individual evaluations 2 times each year for all 37-39 residents, remediation programs, laboratory experiences, and all teaching conferences.

Accomplishments:

1. RRC-Surgery review and site visit - Completed 4/95, result - Full Accreditation

2. Didactic clinical and basic science curriculum - Completely revised - 4 semester topic rotation instituted covering all of surgery in an orderly and complete fashion. Each cycle takes two years and is divided into 72 topics. Each resident goes through the cycle 3 times. Conferences are interactive and resident presented based on assigned textbook readings. A selected expert attending is present to answer questions and give perspective.

3. Surgical specialty conferences - Reorganized integrating breast, colorectal, oncology, trauma and vascular specialties.

4. Visiting Professorships - Established. Include a formal lecture at Surgery Grand Rounds followed by Visiting Professor Rounds with selected case presentations and discussion by the professor with the residents and students alone. The Chief Residents also have the opportunity to interact with the visiting professor either at dinner the night before or at lunch that day.

5. Senior/Chief Resident’s Case Conferences - New conference created to improve application of surgical knowledge and clinical decision making. A selected faculty member presents an clinical scenario to the residents and then they work it through, applying an algorithmic type of approach to the problem. The cases are presented to the residents as unknowns, however, the topics are chosen by the Program Director so there is reasonable variety.

6. Intern conference - For PGY 1 residents, an year long introductory conference run by the chief and senior residents with my supervision. The first set of conferences are to introduce the new residents to the hospital and the institution and to cover basic floor management of patients. These topics include intravenous line placement and management, tubes, orders, professional interactions and service coverage. The remainder of the year is spent reading and discussing Sabiston's text Essentials of Surgery, chapter by chapter.

7. Wednesday Morning Teaching Conferences - Wednesday morning is designated a non-operating, teaching day and all elective general surgery time was moved to other slots. Our formal teaching conferences are concentrated in this time with minimal distractions.

8. Technical Skills Laboratories - Developed to teach technical skills in a hands-on fashion. Each Wednesday afternoon 4 to 6 residents spend several hours in the Endosurgical Education and Research Center laboratory in Ross Hall performing skill exercises on both inanimate and animate models. A graded program has been designed which begins with techniques of suturing, tying and cutting and progresses to anastomotic techniques, endoscopy and laparoscopy. The program is based on a gradual, incremental approach to increasingly complex tasks. Individual progress and problem areas are identified and tracked on Skill Development Assessment Sheets. This assists in planning upcoming training sessions and serve as an indicator of the individual's readiness to perform advanced laparoscopic procedures in the Operating Room. This program also addresses related issues such as equipment trouble-shooting, familiarity with the instrumentation and use of the sophisticated video equipment. Each resident gets to the lab about once each quarter. This program is under the direction of Jonathan Sackier, M.D., Director of Surgical Endoscopy and Endosurgery.

9. In-house written examination - This examination simulates the Qualifying (written) Exam and the In-Training Exam (ABSITE) of the American Board of Surgery. Our residents generally only take one exam each year, the ABSITE in late January. Results vary and I felt this was not a sufficient instrument to gauge our resident’s knowledge base. Therefore this exam was written and administered for the first time on 12/2/95 to give the resident’s additional experience in this type of examination and to enhance our evaluation of their progress. Once the resident completes the exam and hands in the answer sheet they are given the correct answers and take the questions home to be used for study.

10. Mock Oral Board Examinations (MOBEs) - Initiated in 1993, this program simulates the Certifying (oral) Examination of the American Board of Surgery, the final step before board certification. Most residents do not have formal experience in taking oral examinations and this tends to be the most anxiety-provoking process in the training of a surgical resident. We therefore started a program of twice-a-year MOBEs for all senior and chief residents. The exam is given in the same fashion as the real one with sessions of 20-30 minutes with each examinee being questioned by 2 examiners in each of 3 sessions. A wide range of topics is covered. Each resident is scored on an evaluation form developed for this purpose. Immediately after the exam, all faculty and residents gather together and the questions posed are discussed along with anonymous examples of incorrect approaches. The residents are then excused and the faculty discuss each resident’s performance. Detailed feedback is given on an individual basis to each resident by me.

11. Faculty advisor program - This program was begun in 1992. Each resident is assigned a Faculty advisor when they enter the program. They may change advisors if they choose. They meet with their advisor at least twice each year for discussion of progress, problems and career plans. This is in addition to their twice yearly meetings with me.

12. Surgical Education Committee - This committee was reconstituted when I was appointed Program Director. It is composed of all affiliated institution residency directors, the Department Chairman, the Division of General Surgery Director, several appointed members and a Chief resident. The Committee takes an active role in administration and review of the residency program. Quarterly meetings are held where all aspects of the program are discussed.

Direct teaching activities

1. Resident lectures - ~ 2 per month.

2. Student lectures - ~ 4 per month.

3. Formal teaching rounds -Tuesday and Thursday morning from 0730-0900.

4. Wednesday conferences – Morbidity and Mortality, Grand Rounds,

Basic Science Conference, Senior Case Conference

5. Student advisor - 4-6 students at a time

6. Resident evaluations - 39 residents
