How To Join Our Program

Joining Our Program is Easy!
Thank you for you interest in our Bariatric Surgery Center of Excellence Program. By following the steps below you will begin the application process and your journey towards a new, healthier, you.
1. Click here to learn more about bariatric surgery.
2. Determine your Body Mass Index (BMI), a measure of your weight with your height taken into account. Use our BMI Calculator.
If you are interested in a medical (non-surgical) approach to weight loss, go directly to step #4
3. Download our PDF file for surgical program specifics including insurance eligibility requirements, types of surgery, resources and the appointment processes.
4. Mail or fax the preliminary application (PDF) to us:

Join us at our Jordan Hospital affiliate in PLYMOUTH, MA –The first Thursday of every month from 6:00 - 7:00pm in the Funkhouser Conference Room.
Please register by calling 617-636-7682.
You may also come to our HOWL SEMINAR where you can learn from our clinical team members and hear first hand from our patients who have undergone surgery.
The HOWL Seminar is held the 3rd Monday of each month from 6:00-7:00pm in the Wolff Auditorium at Tufts Medical Center.
Friends and family are also welcome to attend these free seminars – no children please. |
