Primary Care Boston recertified as a Level 3 Patient Centered Medical Home

Being a patient centered medical home means that we have reached (and exceed) the patient centered care requirements of the National Committee for Quality Assurance.

The requirements that we’ve met help you live a happier and healthier life. They include:

  • Having physicians who develop ongoing relationships with patients and who are trained to provide first contact, continuous and comprehensive care
  • Having physicians lead the practice and take responsibility for ongoing care of patients
  • Providing all of your primary care and arranging for specialty care when needed
  • Coordinating care across the Tufts Medical Center health system
  • Having strong quality and safety
  • Making it easy for you to schedule appointments quickly
