Determination of Need Community Health Initiative funding opportunity (due January 5, 2024)

On behalf of the Medford Surgery Center, LCC and Tufts Medicine MelroseWakefield Hospital and Lawrence Memorial Hospital, our fourth and final year of funding is being offered to the community to fund $114,938 in Community Health Initiatives (CHI). This funding is available per the Massachusetts Department of Public Health (DPH) Determination of Need (DoN) Regulation, in association with DoN Application #18060613-AS, for the Medford Surgery Center, LCC. (a joint venture of Tufts Medicine MelroseWakefield Hospital, Lawrence Memorial Hospital and Shields Healthcare).

The funding is to support policy, system, and/or environmental change that will improve health, build awareness and impact access to care in three at-risk populations:

  1. School-aged children (grades K-8)
  2. Older adults (aged 60+)
  3. At-risk community residents (through a focus on the stigma associated with mental illness/mental health that prevents broad access to care)
