Specialty Pharmacy Patient Rights

You have the right to:
  • Select those who provide you with Pharmacy services
  • Receive instructions on handling drug recall
  • Speak with a health professional anytime during your treatment and care
  • Receive instructions on the safe disposal of drugs in compliance with state and federal laws and regulations
  • Receive information about the Specialty Pharmacy Patient Management Program
  • Know about philosophy and characteristics of the Specialty Pharmacy Patient Management Program
  • Have personal health information shared with the Specialty Pharmacy Patient Management Program only in accordance with state and federal law
  • Identify Specialty Pharmacy Patient Management Program’s staff members and their job title, and to speak with a supervisor of the staff member if requested
  • Receive information about changes in or termination of the Specialty Pharmacy Patient Management Program
  • Decline participation, revoke consent or dis-enroll in the Specialty Pharmacy Patient Management Program at any point in time

You have a responsibility to:

  • Respect the rights of Pharmacy personnel
  • Notify your Physician and the Pharmacy with any potential side effects and/or complications
  • Notify Tufts Medical Center Specialty Pharmacy via telephone when medication supply is running low so refills can be shipped to you promptly 
  • Submit any forms that are necessary to participate in the Specialty Pharmacy Patient Management Program to the extent required by law
  • Give accurate clinical and contact information and to notify the Specialty Pharmacy of changes in this information
  • Notify your treating provider of your participation in the Specialty Pharmacy Patient Management Program, if applicable.
