Please bring the following with you to your initial appointment:

• Hospital blue card.
• Insurance card.
• Prescription from your referring physician.
• If your insurance requires a referral, it is your responsibility to contact your doctor's office prior to your initial appointment.
• If your injury is work related, you must provide us with the worker's compensation information and contact telephone numbers prior to your initial appointment.
• If your injury is related to a motor vehicle accident, you must provide us with the name of the insurance company and contact telephone numbers.
• Insurance co-pays are expected at the time of each visit.
• Prior to initial appointment, please call Registration at 617-636-0084 to ensure all contact and insurance information in correct.
